Abstract

Objective: We hypothesized FM in migraine hospitalizations were associated with poor outcomes [loss of function, discharge other than home, higher length of stay (LOS) and cost] in compared to patients without FM.

Background: The increase in migraine frequency – with the shift towards chronicity – promotes an enhancement of the hypersensitivity or triggers pain of fibromyalgia and myositis (FM); Co-morbidity between FM and Migraine involves a higher state of painful somatic hypersensitivity with respect to one condition only.

FM based on 2010 American College of Rheumatology diagnostic criteria was more prevalent among patients with chronic migraine.

Design/Methods: We performed a population-based analysis of the Nationwide Inpatient Sample (NIS) (years 2003–2014) in adult hospitalizations for the Migraine to determine the prevalence, cost, LOS, loss of function (disability), and discharge disposition associated with FM patients in compare to patients without FM, using ICD-9-CM codes (migraine: 346; FM: 729.1). We performed weighted analyses using Chi-Square, student t-test, and Cochran-Armitage trend test.

Results: Between years 2003 to 2014, of the total 596,231 migraine hospitalizations; 27,760 presented with FM with a significant increase in trend from 3.07% in 2003 to 6.21% in 2014 (p<0.0001). FM patients had higher frequencies of major/extreme loss of function (8.12% vs. 6.22%; p<0.0001), transferred to short-term hospital/skilled nursing facility/intermediate care/long-term care facility (5.15% vs. 4.05%; p<0.0001), and lower frequencies to Discharge to home (94.85% vs. 95.95%; p<0.0001) in compare to non-FM. FM admissions were associated with higher LOS (+1 day; p<0.0001) and cost of hospitalization (+ $2300; p<0.0001).

Conclusions: FM risk assessment and prevention of the chronic migraine will reduce the burden of disability and improve the outcomes in concurrent FM with migraine hospitalizations.
