Embedded Image

This issue highlights a Clinical Reasoning article featuring a 22-year-old man with progressive bilateral visual loss. A Pearls & Oy-sters article showcases fatal brain edema as a rare complication of CACNA1A-related disorder. A Teaching NeuroImage focuses on Ondine curse syndrome and a Teaching Video NeuroImage looks at enhanced ptosis in bedside MG testing.

Pearls & Oy-sters: Fatal brain edema is a rare complication of severe CACNA1A-related disorder

A 5-year-old girl presented with hypotonia and eye movement abnormalities. This case highlights a severe presentation of the rare CACNA1A-related disorder.

Clinical Reasoning: A 22-year-old man with progressive bilateral visual loss

This case discusses considerations to be made—such as looking for cryopyrin-associated periodic fever syndrome—in chronic aseptic meningitis.

Page 625

Teaching NeuroImages: Ondine curse syndrome caused by dorsolateral medullary stroke

A 68-year-old man with recurrent hypercarbic respiratory failure is presented. Lesions in the dorsolateral medulla may lead to Ondine curse syndrome.

April 14, 2020, issue

This issue presents a middle-aged man with rapidly progressive ataxia and vitiligo in a Clinical Reasoning case. In a Residency Training article, there is a practical guide for students planning a future neurology career. In a Pearls & Oy-sters article, the rare presentation of rapid progression of prion disease with transverse myelitis is presented. In a Teaching Video NeuroImage, a case of vagus nerve schwannoma is shown.

Residency Training: A practical guide for medical students who are planning a future in neurology

The article provides advice for medical students at every level of training on preparation for a neurology residency application and a career in neurology.

Clinical Reasoning: A 47-year-old man with rapidly progressive ataxia and vitiligo

This case discusses autoimmune cerebellar ataxia as an important differential diagnosis in rapidly progressive cerebellar disease.

Page e1664

Pearls & Oy-sters: Rapid progression of prion disease associated with transverse myelitis: A rare presentation

A 57-year-old man had rapidly progressive Creutzfeldt-Jakob disease with spinal involvement and the case showcases its consideration in transverse myelitis. The patient presented with progressive clumsiness, gait instability, and bowel and bladder incontinence.

Teaching Video NeuroImages: Vagus nerve schwannoma: A trigger for cough

This case presents a man with a cough triggered by schwannoma.

April 21, 2020, issue

This issue features a Clinical Reasoning article discussing a teenage girl with ataxia, oscillopsia, and behavioral changes. A Pearls & Oy-sters article highlights isolated oculomotor nerve palsy due to pituitary apoplexy. The first Teaching NeuroImage shows pseudopathologic brain parenchymal enhancement, and the second highlights extrapontine osmotic demyelination in hypernatremia.

Clinical Reasoning: A 16-year-old girl with ataxia, oscillopsia, and behavioral changes

Opsoclonus associated with ovarian teratoma is treatable with immunosuppressive therapy. A 16-year-old girl presented with vertigo, tremulousness, and gait instability.

Page 713

Pearls & Oy-sters: Isolated oculomotor nerve palsy due to pituitary apoplexy missed on CT scan

A 70-year-old man with a history of type II diabetes and other medical issues presented with binocular diplopia and right-sided ptosis after treatment for headache. Pituitary apoplexy is rare but potentially fatal and patients often present with retro-orbital headache possibly accompanied by visual field deficits.

Teaching NeuroImages: Pseudopathologic brain parenchymal enhancement due to vascular compression in parotid tumor

The case discusses rarely described pseudopathologic brain parenchymal enhancement in a 74-year-old man.

Page e1778

Teaching NeuroImages: Extrapontine osmotic demyelination in hypernatremia

Few cases of extrapontine osmotic demyelination are reported in hypernatremia.

April 28, 2020, issue

This issue features a Journal Club article reviewing a study by Burneo et al. looking at the relationship between stroke incidence and new-onset epilepsy in the elderly. A Mystery Case article presents anti-NMDAR encephalitis with overlapping demyelinating syndrome. A Teaching Video NeuroImage illustrates a case of enhanced ptosis in myasthenia gravis.

Journal Club: Risk of new-onset epilepsy and refractory epilepsy in older adult stroke survivors

This article reviews a study by Burneo et al. on new-onset epilepsy in older patients with stroke.

Mystery Case: Anti-NMDAR encephalitis with overlapping demyelinating syndrome

This case describes a 31-year-old man who presented with subacute onset of headache, left-sided numbness, and anterograde amnesia. The patient showed overlapping demyelinating syndrome with anti-NMDAR encephalitis.

Page e1866

Teaching Video NeuroImages: Useful bedside testing for myasthenia gravis: The phenomenon of enhanced ptosis

A 62-year-old man with myasthenia gravis presented with enhanced ptosis.
