Notable in Neurology This Week

This issue features an article that identifies metabolites associated with incident stroke; another examines disease trajectories in patients with Alzheimer disease or dementia with Lewy bodies in the years before death. A featured Research Article investigates the association between serum antioxidant vitamins and carotenoids with Alzheimer disease dementia.

Research Articles

Progression to Dementia or Reversion to Normal Cognition in Mild Cognitive Impairment as a Function of Late-Onset Neuropsychiatric Symptoms

Embedded Image

This study investigates whether the presence of mild behavioral impairment (MBI) improves the prognostic specificity of mild cognitive impairment (MCI) for progression to dementia. Patients with MCI and MBI were more likely to progress to dementia than were patients with MCI alone. Assessing cognition and behavior results in more accurate dementia risk assessment than when only cognition is considered.

Prescribing Trends for Valproate Among Pregnant Women: A Cross-sectional Study in 2013 and 2016 Using the French Health Insurance Database

Embedded Image

This cross-sectional study describes prescribing trends for sodium valproate (VPA) before and after the European Medicines Agency recommended the disuse of VPA during pregnancy. Among pregnant women with epilepsy and those with bipolar disorder, exposure to VPA decreased between 2013 and 2016.

Effectiveness of a Self-Management Program to Improve Cognition and Quality of Life in Epilepsy: A Pragmatic, Randomized, Multicenter Trial

This pragmatic trial evaluated the effectiveness of the Home-Based Self-Management and Cognitive Training Changes Lives (HOBSCOTCH) intervention for people with epilepsy. In-person delivery of HOBSCOTCH improved cognition and quality of life. Virtual delivery of HOBSCOTCH was feasible but did not significantly improve outcomes.

From editorialist Hopp: “[It] … makes sense to engage and empower patients in their own care as an active role in treatment can improve self-efficacy and lead to improved quality of life.”
